Konocti Connections is dedicated to providing support that changes with the consumer's need, but consistently meets their level of satisfaction.
The mission of Konocti Connections is to provide consumers with the assistance they need to maintain the independent living arrangement of their choice.
We possess the longevity, history, and above all, the experience to offer consumers a quality of support that is achieved only through 30 years of direct service.
As providers, we work with consumers to assess their situations and abilities to make an educated choice, while at the same time, facilitating self-reliance at a level based on individual need, and always mindful of the dignity of risk.
All staff are certified in the implementation of Pro-Act principles when working with consumers who are moving into crisis when unable to manage their life challenges.
Services from Konocti Connections are person-centered, at a 1:1 ratio. The consumer controls the direction his/her life takes, and the choices that direction entails. In that sense, our clients:
Determine the areas of support where they will receive assistance.
Determine the amount of time they will spend with their KC Community Resource Specialist.
Set the schedule of appointments with the KC/CRS, consistent with their daily routine.
Typically, support services begin in the home, but as in any household, individual needs extend the necessity of support into all areas of the community. Our role is to assist our clients and their families with accessing a vast array of generic services and successfully integrating their communities. Services include access to:
Banks (includes budgeting, bill-paying)
Healthcare care Providers locating/accessing a primary physician, or dentist (including out-of-county and accompanying parents with children to UCSF)
Pharmacies
MediCal and MediCare Programs
Social Security

In-Home Support Services
