For individuals who choose to live independently, Konocti Connections (KC) offers a variety of services and supports. The mission of Konocti Connections is to provide consumers with the assistance they need to maintain the independent living arrangement of their choice.

Services from Konocti Connections are person-centered at a 1:1 ratio. The consumer controls the direction of his/her life and the areas of support in which they desire assistance. Consumers determine the amount of time they spend with their KC Community Resource Specialist, and set the schedule of appointments with KC, consistent with their own routine. Among the many services provided are assistance in setting up medical and dental appointments, accompanying clients to appointments and on shopping trips, and aiding clients with court and legal matters.

The role of Konocti Connections is to assist our clients and their families with accessing a vast array of generic services and successfully integrating with their communities. As providers, we work with consumers to assess their own situations and abilities and to facilitate self-reliance at a level based on individual need.
